Service Description

Evaluation and medication management for patients who may require a controlled medication as part of an appropriate treatment plan. This visit may include: • Comprehensive medical evaluation and review of your current symptoms • Review of medical history, medications, allergies, and previous treatment • Assessment of the risks, benefits, and appropriateness of controlled medication therapy • Review of previous prescriptions and treatment records when applicable • Prescription monitoring program (PDMP) review when required • Review of relevant laboratory or diagnostic results when medically appropriate • Medication adjustment, continuation, or initiation when clinically appropriate • Monitoring for medication effectiveness, side effects, misuse, dependence, or other safety concerns • Discussion of non-controlled medication and non-pharmacologic treatment alternatives • Follow-up monitoring as clinically and legally required Please select this service if you are seeking evaluation or ongoing management of a medication that is classified as a controlled substance.
